new account is opened.

information necessary to deliver quality products and services
to our customers or as required by law.

We

experiences or transactions with you or your accounts (e.g.
account balances or payment history) or collected information
(e.g. credit reports or asset and income information) with our
affiliates.

or services to you.

affiliated companies, nonaffiliated third parties or
joint-marketers, other than that permitted by law, you may
exercise your right to opt out of this activity. To opt
out, please call us toll-free at 1-800-448-PROV (7768) or
write to us at the address listed below. When writing to us,
please make sure that your request clearly indicates that you
do not wish to have your collected information shared and that
your correspondence clearly identifies you, your Social
Security number and your affected account numbers. If you have
previously informed us of your preference to opt out, you do
not need to do so again. We will honor your choice of
restricting information sharing with Provident’s affiliates.
However, this restriction does not apply to your account
transactions and experiences with us, nor does it apply to our
identification information verification
process.

the opt out directive by a joint owner as applying to all of
the associated joint owners. However, should any joint owners
have other accounts with us independent of you, your request
